SAGE Private Services can help with in-home care, adult daytime care, geriatric care management, home delivered meals and more!



 


For more than 50 years, SCI has been supporting our community by helping its older adults age successfully. Our SAGE Private Services can help with in-home care, adult daytime care, geriatric care management, home-delivered meals and more. SAGE provides high-quality services to help people 55 years of age and older, as well as those with short- or long-term disabilities. With over 50 years of experience helping people in our community age successfully, SAGE is the area expert on providing services that extend and enhance independent living for older adults.

Home Delivered Meals
Our home delivered meals are hot, delicious, and affordable. Our meals are designed by a registered dietician and are low sodium. They are prepared fresh every weekday for people who cannot or do not want to prepare meals for themselves. Ideal for those whose needs are short-term (e.g., people recovering from surgery) or long-term, hot meals are delivered Monday through Friday. Healthy frozen meals are available to supplement nights and weekends. Most importantly, a caring person hand-delivers our meals to the door each day and can call for help if the client has critical needs or requests additional assistance.


In-Home Care
Nursing services – medication set-up and administration, wound care, blood draws, diabetes management, catheter care, and more, performed by a licensed RN or LPN
Personal care – bathing, grooming, medication reminders, etc. provided by a Certified Nursing Assistant
Homemaker services – laundry, cleaning, errands, and simple meal preparation, etc.
Companion services – conversation, games, exercise, and friendship for the senior adult who needs more socialization or cannot be left alone.


Adult Daytime Care
Adult Daytime Care Centers are located in Savannah and Hinesville for adults eighteen or older with cognitive, developmental or physical impairments who need assistance and supervision throughout the day. The Daytime Care Director, staff RN, LPN, Activity Director, and client’s family work together to create a customized care plan for each ADC client. Our Centers provide social interaction, healthy meals, medication administration, daily living assistance, and physical activity. Professional and compassionate caregivers provide their clients’ families the confidence of knowing their loved one is safe and engaged in meaningful ways while offering respite to caregivers. Clients may choose all-day or half-day care, Monday through Friday in Savannah and Tuesday through Thursday in Hinesville.



Geriatric Care Management
The Geriatric Care Management program defines a course of action to help with decisions and tasks related to the care of aging loved ones. The program provides a one-time professional assessment and/or ongoing care management services based on the needs of the client and his or her family. The Geriatric Care Manager (GCM) comprehensively assesses the physical, emotional, cognitive, social, environmental, and financial care needs of the client as appropriate. Our GCM is a licensed clinical social worker with over 20 years of experience with eldercare issues, family dynamics, gerontology, and psychology in diverse healthcare settings. After identifying the presenting concerns, she navigates through caregiving issues, finds solutions, and executes a plan that best works for the family.
